At Montreal, on June 16, 2001, at the age of sixty-three years, suddenly at home. Predeceased by mother and father Anna and Ivan Kuzyshyn, survived by sister Stefania Mushinski. Private family service will be held at Mont-Royal cemetery. May he rest in peace. Montreal Gazette, June 21, 2001
